Bright Mosaic Therapy Centers (“Bright Mosaic”, “we”, “us”) provides ABA therapy, speech therapy, and occupational therapy for children at centers in North Texas. This policy explains what information our website, www.brightmosaic.org, collects, how we use it, and the choices you have.
1. Information you give us
Contact form. When you ask about services through our contact form, we ask for your name and phone number, and optionally your email address; your child’s date of birth; your child’s primary insurance; whether a medical provider has diagnosed your child with autism; the area you are interested in; and how you heard about us. The form also records which page of our website you came from, so we know which center or service you were reading about.
Job applications. When you apply for a position, we ask for your name, phone number, email address, home address, resume, cover letter, and the times you are available to interview, along with the position you are applying for.
Phone and email. When you call or email us, we receive the information you choose to share.
Providing this information is voluntary. We use it only to respond to you, to enroll children in our services, and to consider job applications.
2. How the forms are handled
Our forms are provided by Jotform. What you enter is sent directly to Jotform over an encrypted connection and is not stored on our website’s servers. The contact form uses Jotform’s HIPAA-compliant settings. Only the Bright Mosaic staff who respond to inquiries and applications can see what you submit.
We never send the contents of our forms to advertising or analytics providers.
3. Information collected automatically
Analytics. We use Google Tag Manager and Google Analytics to understand how visitors find and use our website, so we can improve it. When you visit our website, Google Analytics sets cookies and receives technical information about your visit, such as the pages you view, your browser and device, and the general location of your device. When you contact us through our website, tap a phone number, or apply for a job, we record that it happened and which page it came from. We do not share what you wrote or said, and Google Analytics does not receive the contents of our forms.
Advertising measurement. We use Google Tag Manager and Google Ads to learn which of our ads lead families to contact us. When you visit our website, Google may set cookies and receive technical information such as your IP address, browser, and the pages you visit. When you contact us through our website or tap a phone number, we record that a contact happened and which page it came from. We do not share what you wrote or said, and we do not use your visits to build advertising audiences or to show you ads on other websites.
Maps. Our location pages show maps provided by Google Maps. When a map loads, Google receives your IP address and browser information.
Hosting. Our website host keeps standard server logs, such as IP addresses and browser types, for security and performance.
4. Cookies and your choices
Our website uses cookies that it needs to work, and the Google services above set cookies of their own. You can block or delete cookies in your browser settings; the website will still work. You can also stop Google Analytics from measuring your visits with Google’s opt-out browser add-on at tools.google.com/dlpage/gaoptout, and manage how Google uses your data for ads at myadcenter.google.com.
5. How we share information
We do not sell personal information, and we do not share it for anyone else’s marketing. We share information only with the service providers that help us run our website and respond to you (such as Jotform, Google, and our website host), when the law requires it, or to protect the safety of the children and families we serve.
6. Children
Our website is for parents, caregivers, and job applicants. It is not directed to children, and we do not knowingly collect information from children under 13. Information about a child comes to us from a parent or guardian, for the purpose of enrolling the child in our services.
7. Health information
Once a child is enrolled, the child’s health information is protected by HIPAA and is handled under our privacy practices for clients, not under this website policy.
8. How long we keep information
We keep enrollment inquiries for as long as we need them to respond and to complete enrollment, and job applications for as long as we need them for hiring. After that, we manage them under our internal retention policies.
9. Security
Our website uses encryption (HTTPS) for every page and form, and we limit access to what you submit to the staff who need it. No method of transmission or storage is perfectly secure, but we work to protect your information.
10. Your requests
You can ask us to access, correct, or delete the information you have submitted through our website by contacting us below.
11. Changes to this policy
We may update this policy. When we do, we will post the new version on this page and change the date at the top.
